Provides self-sticking label sections at top with die cut rounded corners that includes a vial label plus warning labels that peel-off as one unit, yet warning labels may be easily detached. Lower portion of form is bond paper for patient education messages. Laser printer compatible label forms are 8 1/2 X 11 size including patient counseling message area.
The warning labels are separated with a vertical perforation which leaves 3 on the left connected and 3 on the right connected.
Replacement for the A54 label (A-54), product ID 5839.
